All disability-related academic support services from the Office for Students With Disabilities (OSD) are based on your current diagnosis and correlated limitations that may impede your academic processing.
Step 1: Register in person.
Before you receive any academic support, you must register in person with the Office for Students with Disabilities (OSD) by completing an intake form.
Step 2: Complete OSD's consent form.
Once you have registered, OSD will ask you to complete a form that provides consent for OSD to obtain current written disability verification directly from your treating professional.
Step 3: Review disability assessment guidelines and documentation required for:
- Learning disabilities
- Psychological disabilities
- All other medical conditions, which require the following documentation:
- Clearly stated disability diagnosis
- Medical history relevant to the disability
- Functional limitations described in detail (e.g. specific symptoms, frequency, severity, stamina, medication side effects, etc.)
- A list and summary of any testing used in the evaluation (students who are deaf or hard of hearing should include a copy of their current audiogram)
- Estimated end date, if your condition is temporary
- Documentation typed or printed on official letterhead and signed by a treating professional qualified to make the diagnosis
After review, OSD will determine the most appropriate accommodation to meet your needs on campus and will contact you.
